I believe it is the unsliced only for the Personal Allowance and top sliced for all others such as PSA

No need for the form. Just phone them and they will help you out. You need to prove that you have the R03 (or equivalent) but the CII/PFS learning certificate sufficed. There is a fee (can't remember how much). We exchanged docs via email
